Job Description: Full time on roll / Consultant - (Visit Basis)
· To assess the patients’ needs and formulate a personalized care plan based on comprehensive assessment
· To deliver outstanding Physio care guided by an evidence - based care plan and ethical practices
· To monitor the care delivered, and provide necessary Physiotherapy to facilitate better recovery
· To provide goal - based care for patients, resulting in better outcomes Patient and Family Satisfaction
· To deliver home healthcare with a human touch, driven with empathy and compassion.
· To maintain effective communication with all stakeholders involved in patient care.
· To maintain and monitor feedback from patients and their family about the services.
· Clinical data review, analysis and intervention to improve clinical care for patients.
Profile Description:
Qualification - BPT / MPT
Experience - 1 - 5 years’ experience.
Good communication skills in English, Hindi, and local language
Must be passionate about addressing the needs of patients with chronic ailments.
Prior experience in home healthcare will be an added advantage
